5. Accident / Incident Reporting

At Bolton HC, the safety of our members is of paramount importance to us. Although we will do all we can to minimise accidents from happening, we understand that with the nature of any sport there may be times when accidents may occur.

Our accident / incident reporting process is in place to ensure we effectively protect our members and can offer the appropriate care and support where necessary.

Please find below the Accident / Incident Report Card if there is something you want and need to report, please return any completed forms to Fiona Clarke (welfare@boltonhockeyclub.co.uk)
